Cross contamination could be cause by carrying raw food which might be contaminated or touching other food, ready to eat meals or surfaces can also cause cross contamination.
There are some juices from raw food dripping on other food items which can cause this. Food handlers who have dirty hands and who are touching food can cause cross contamination.
Using the same utensils or equipment to handle raw and cooked food.
All of the above can cause cross contamination.
