In the book The Giver, the process of the release of a child is that a doctor injects them with a fatal substance. They twitch, die, and get sent through a garbage chute to God knows where. While the people in the "perfect" town are told they get released into the outside of the town, the only the doctors and the Giver (and soon Jonas) know that they are killed.
